Instructions for collecting healthcare insurance claim records

The compensation file basically includes

– Paper of claim.
– Medical documents: prescriptions, medical records, results, indications of tests, x-rays, discharge certificates, surgical certificates, appointment of physiotherapy set, etc.
– Payment documents: medical bill, examination fee …
– Other documents: Accident report, driver’s license, vehicle documentation (in case of accident), referral (in case of transfer).

Depending on the case, there will be specific documents of that type of treatment. As follows

In case of dental examination

– Tooth treatment vouchers.
– Dental films (in case of pulp, extractions, fillings, etc., films are required)
– Money bill for dental treatment.
– Other papers …

In case of outpatient examination

– Prescription, medical book (stamped by clinic / hospital)
– Results of tests, x-rays … (if any)
– Payment vouchers such as invoices, receipts for medical examination and drug money
– Detailed list of medical examination expenses (if the invoice does not clearly show the expenses for medical examination)
– The appointment letter of physiotherapy and exercise schedule (if there is a physical therapy episode)
– Other papers …

Hospital / surgical case

– Paper of discharge (For cases of hospitalization).
– Certificate of surgery (For surgical cases)
– Payment vouchers such as invoices, receipts for medical examination and drug money
– Detailed list of medical examination expenses (if the invoice does not clearly show the expenses for medical examination)
– Paper of transfer (If having transferred hospital)
– Other papers …

Case of Accident

– Accident report
– Driver’s license, vehicle certificate (in case of traffic accident)
– Medical documents: prescription, medical records. Laboratory results, x-rays …
– Payment vouchers: invoices, receipts ..
– Detailed list of medical examination expenses (if the invoice does not clearly show the expenses for medical examination)
– Other papers …

Death case

– Death Certificate
– Certificate of discharge (if any is in hospital)
– And other documents as instructed ..

NOTE:

– A tooth treatment note must clearly state: full name of the insured, date of dental examination, cause of extraction, filling material, rooting procedures, full name of the treating physician, carpentry of the room dentistry, hospital on the tooth treatment note.
– The receipt, the receipt for the examination and the medicine must be the original. A bill of over 200,000 VND must be changed to VAT invoice. The bill of examination + test amount of over VND 200,000 must be changed into a VAT invoice.
– Prescription / medical examination book must clearly state: Full name of the insured, date of examination, diagnosis, name of drug, clear quantity of medicine and must have signature, full name of treating physician, rustic disease hospital / clinic on prescription.
– According to the regulations of the Ministry of Health, the time to buy medicine is within 5 days from the date of medical examination.
– Test results, X-ray … can submit a copy. Tests that are not related to the disease and tests that have Normal results are not paid.
– In the case of an occupational accident at the company, there must be a seal of the company on the report of the insured.
– In case of a traffic accident, a driver’s license is required.
