One of the most common problems with health insurance claims is not the treatment itself — it is missing documentation.

After visiting a doctor or hospital, make sure you keep the relevant documents.

Here are 7 important types of documents:

1. Medical examination record

This should normally show the diagnosis and treatment recommendation.

2. Prescription

Keep the original prescription or valid electronic medical prescription.

3. Medical test results

Examples include blood tests, ultrasound, X-ray, CT scans or other diagnostic results.

4. Medical treatment records

These may include treatment sheets, medical reports or other documents showing what treatment was provided.

5. Hospital discharge summary

If you are hospitalized, keep the discharge summary or medical report.

6. VAT invoice

Keep the original VAT invoice issued by the medical facility.

A simple payment receipt or retail receipt may not be sufficient for a claim.

7. Detailed statement of medical expenses

This helps show exactly what services, medicines and treatments were charged.
