So here is a simple checklist you can save.

BEFORE TREATMENT

☐ Check whether the medical facility is in your insurer’s cashless network

☐ Check whether your insurance plan covers the type of treatment

☐ Bring your insurance card

☐ Bring your identification document

☐ Check whether the medical facility has any special cashless procedures

DURING TREATMENT

☐ Keep your medical records

☐ Keep prescriptions

☐ Keep test orders and results

☐ Keep imaging results

☐ Keep treatment records

☐ If hospitalized, keep the discharge summary

☐ If surgery is performed, keep the surgery certificate / operation report

AFTER PAYMENT

☐ Obtain the required VAT invoice

☐ Obtain the detailed statement of medical expenses where applicable

☐ Check that your name and personal information are correct

☐ Keep copies/photos of all documents

☐ Prepare an accident statement

☐ Keep your driving licence if applicable

BEFORE SUBMITTING THE CLAIM
