Health Declaration for Tam Binh: 4 Questions and Key Undertakings

5 COMMON MISTAKES WHEN SUBMITTING A HEALTH INSURANCE CLAIM

Filling in the insurance application form is a step many families rush through, yet it determines whether your benefits will be protected later. The Tam Binh health declaration has four questions that look simple, but each one deserves an honest and specific answer.

📋 What does the Tam Binh health declaration ask?

The Bao Viet Tam Binh application form applies to individuals, families and organisations with fewer than three employees, and requires a separate declaration for each insured person. Before filling it in, you can review the product structure in What Is Bao Viet Tam Binh? to understand each benefit section.

  1. Medical history over the past three years. Asks whether any insured person has ever been examined, received medical advice, been prescribed treatment, or been treated at a clinic, hospital, nursing home or similar medical facility.
  2. Current health status. Asks whether anyone is being monitored or treated for an injury or illness, has unstable health symptoms, or has been advised to undergo treatment within the next 12 months.
  3. List of serious illnesses in the past three years. Asks whether anyone has had and/or been treated for any condition on the list in the insurance rules.
  4. Claims history and insurance rejections. Asks whether anyone has had a claim settled by Bao Viet under a similar health insurance policy; been refused cover or renewal by any insurer; or been accepted with special additional terms.

Conditions usually not required in Question 1

According to the application form, some common conditions may not need to be declared, including: digestive disorders, acute diarrhoea, constipation, food poisoning, common cold, flu, viral fever, outer ear infection, acute nose/throat inflammation, upper respiratory tract infection (excluding sinusitis, tonsillitis, Eustachian tube (VA) inflammation, middle ear infection and chronic diseases), hand-foot-mouth disease, chickenpox, conjunctivitis, stye/chalazion, allergies, prenatal check-ups, contraception, appendectomy, childbirth, dental treatment, gingivitis, vaccinations, and injuries fully recovered with no further monitoring or treatment needed.

Serious illnesses listed in Question 3

  • Central nervous system (brain) diseases, Parkinson’s, other nervous system degeneration, memory loss, coma, cerebral palsy, paralysis.
  • Diabetes; lung failure, pneumothorax/pleural effusion, chronic respiratory failure.
  • Cerebrovascular disease, stroke (brain haemorrhage/arterial hardening); heart failure, myocardial infarction, heart surgery.
  • Hepatitis, cirrhosis; kidney failure, kidney atrophy, kidney stones, dialysis; pancreatitis; bone marrow transplant.
  • Lupus, tuberculosis of all types, leprosy; tumours of all kinds, cancers of all kinds, bone marrow failure, leukaemia.
  • Sexually transmitted diseases, immunodeficiency syndrome; congenital diseases, hereditary diseases, genetic malformations, physical disabilities, Down syndrome.

If you answer YES to any question, give details: the insured person’s full name, disease name, date of diagnosis, treatment method, current condition and treatment facility. For Question 4, also add the insurer’s name, whether cover was refused or accepted with special additional terms, and the reason.

✍️ Four undertakings to note when signing the application

  • Health warranty: confirm you are currently in full health; no neurological, mental or leprous disease; not currently undergoing treatment for illness or injury; no permanent disability of 80% or more.
  • Information warranty: confirm the declared information is complete and truthful to the best of your knowledge, and that you will notify Bao Viet immediately of any relevant changes; confirm you have received the product document set (insurance rules, benefit schedule, contract sample), been fully advised, and understand the contents of the application.
  • Consequences of declaration: if the duty of honest and full disclosure is not fulfilled, Bao Viet reserves the right to reduce its liability in whole or in part for a claim; if non-disclosure is intended for insurance profiteering, the contract may be cancelled from its effective date.
  • Personal data: agree to Bao Viet’s personal data protection and processing terms, and allow the collection, processing and storage of data, and access to third parties (including medical staff and doctors) on behalf of the insured, to gather the information needed to assess health status.

⚠️ Does answering “Yes” mean rejection?

Not necessarily. Answering Yes gives the insurer a basis to assess the risk level and make an acceptance decision, which may include additional conditions depending on the case. More worrying is answering No when the truth is Yes: when a claim arises, the company can cross-check medical records, and if dishonesty is found, claim benefits may be reduced or the contract cancelled. 🌏 Notes for foreigners and families with young children

Foreigners joining Bao Viet Tam Binh fill in the same application form, using the passport as identification instead of the national ID card (CCCD); every insured person must be declared separately, including children. ✅ Checklist before signing the application

  • ☐ Read each question carefully and answer for all insured persons, including children.
  • ☐ Prepare medical and discharge records from the past three years to cross-check while filling in.
  • ☐ Record the disease name, diagnosis date, treatment method and current condition if you answer Yes.
  • ☐ Double-check full name, date of birth, ID card/passport number and relationship to the policyholder.
  • ☐ Confirm your choice of additional benefits (personal accident or life & personal accident, outpatient treatment) fits your family’s needs.
  • ☐ Keep copies of the application form and the product document set received.
  • ☐ Ask your advisor about anything unclear before signing, instead of skipping or guessing.
