The insurance application form (GYCBH – Giấy yêu cầu bảo hiểm) is an inseparable part of the insurance contract or insurance certificate. By signing it, you confirm that you understand and agree to everything stated in the form and the product documentation. In other words, the declaration is not just an administrative step — it is the basis for the insurer to assess the risk and determine its liability for future claims.
In the commitment section, the applicant and all insured persons must warrant that they are currently fully healthy in body and mind, fit to take the trip, and that the declaration is made while in Vietnam.
